the goal is to give yourself a heart attack but not die before the character reaches the finishes line, this will be determined by a gauge on the left side (or whichever place u want) of the screen. when the player reaches the goal, the character will be catapulted to x2 x3 x4+++ multipliers just like any other endings of running games. you will start off as a starving African child and for the last stage u'll become a full fledge American that could star in my 600lb life.
i first thought that to give yourself heart attack, the character will have to eat fatty foods (common sense) but what if they eat preservatives instead like corn syrup and red 40 (maybe they can be power ups?). Anyway I doubt anyone else would be interested in this because it's a stupid racist game but I felt like shit for even thinking about this game idea/concept in the 1st place so now u have to know about it 2.
OR since ppl like visual novels/rpg then maybe ull play as a friend of a sewerslider character that at first seems normal but the more u interact with them, the more u start to see signs. i think maybe they (the SewChar) will tell you they want to become a mukbanger (that's why they're eating so much) then you find out that they're tryna off themselves via heart attack.
i think the "setting" will take place in a restaurant/samgyupsal/unlimited friend wings/vending machine (doesn't matter as long as u can stuff ur mouth) and ull talk to them, maybe ur also with ur friends (at some parts in the game) that will be beside them or whatever. I think it'll be good for mental health awareness / ED, if executed properly. you may try to stop them or enable them further, the 2 obvious endings are: they d1e from health complications or they start to recover (i don't like it when the character is portrayed as 'fit' as a sign that they recovered from ED because that's unrealistic).
Maybe the game mechanic is you would order from a menu (the foods will determine ur friend's fate) posed as a card game but the 'ap' for healthier foods is much more expensive while unhealthy foods are within your budget range.
Orrrrr, you're a poor person that wants to help their friend so you arrange lunch boxes for them but your budget is still limited and the ingredients u'll need for the lunchboxes may be gained from buying (healthy ingredients is still more expensive) in a shop. Ur friend will at first suggest about what u should buy but as the game progresses theyd start to notice what u were doing and say things likd, "I won't eat this unless it has (insert high calorie energy drink)" so now u'd also have to balance the nutritional content (if u weren't enabling them).
in that sense, u'll also have to earn money, u can do this by spending your 'time' (energy) but you might skip on feeding them your lunch. Higher paying job eats more time and vice versa.
For replayability + to remove predictions, I think it'd be good if there were various representations of ED, like anorexia (they might request that u add rice cakes to their lunch) or bulimia, etc. The condition of Ur friends are random. The jobs may also be a skill, ex. If u work a certain amount in (this job) then you'll gain access to a higher paying job or a much more balanced job. Of course this also means that the game will get harder until it's practically impossible to win.
Problems 1.) It might become an enabler to people with ED (as it gives ideas) 2.) Game balance
